Contact information: add your actual support email/business details
Legal notice: use the version above, then have the final wording reviewed for your actual product classification

One thing I would NOT put anywhere in these policies

Don't write claims like:

“Cures menopause insomnia”
“Stops night sweats”
“Balances hormones”
“Treats insomnia”
“Clinically proven to make you sleep through the night”

Those are exactly the kinds of objective health claims that require appropriate substantiation, and therapeutic claims can also affect FDA classification
